Are you ready to give your child a head start? We have provided a detailed parent questionnaire below to learn valuable insight into your child’s developmental history.
It includes important up to date information for new patients, consent for treatment, and permission to communicate with other professionals regarding your child. Please contact us if you have questions and how to get started with these forms. To limit wait times please fill out these forms prior to your visit.

After the Patient Intake Form is completed, click on the link below that relates to the service (OT, PT or SLP) your child is receiving and fill out the corresponding form.

Once completed, the intake packet will be automatically sent to [email protected] as an attachment.

At your first visit you will be given a comprehensive developmental assessment where you will have an in-depth conversation with our therapist about your child’s needs.

From there, the therapist will walk you through the plan of care to be provided and schedule on going visits. If you have any questions, please feel free to call us and we will be happy to assist you.
